HOMECARE Management's Targeted Case Management program provides services to individuals, both children and adults, with Developmental Disabilities. Services are delivered by professionally trained case managers who are the cornerstones of our case management services in North Carolina.
The Targeted Case Manager identifies each client's needs, coordinates the team of professionals and resources necessary to meet those needs, provides referrals and monitors the delivery and quality of the services each client receives. They also ensure that services support the health and safety of each individual and reflect their needs and preferences as established in their Person Centered Plan.
These professionally trained case managers are responsible for the overall quality of each client's care, including:
- Coordinating and overseeing the assessment and reassessment of each client's need for services, support and level of care
- Developing the Person-Centered Plan (PCP)
- Submitting requests for authorization and revising the initial PCP as needed
- Referring to appropriate community agencies, arranging for authorized services, and assisting clients in working with those agencies.
- Monitoring the delivery of services and assuring quality and appropriateness of client care
- Coordinating and monitoring the services, support and community resources each client receives, including those provided by friends and family
